If you’re an international medical graduate (IMG) navigating your path to working in the NHS, you’ve likely come across the term “clinical attachment.” And you’ve probably also wondered: “Is it still worth it in 2025?” The short answer? It depends.
Let’s break it down honestly – with insights from the community, changing NHS trends, and what you should consider before making your decision.
What is a Clinical Attachment?
A clinical attachment is a short-term, unpaid observational placement in an NHS hospital. It allows IMGs to shadow doctors, get exposure to NHS systems, and sometimes secure reference letters from consultants.
Historically, it was seen as a critical step for building your UK clinical experience – especially before applying for your first job post-PLAB.
Why Some IMGs Say It Still Works
Despite the changing landscape, many doctors still find value in doing an attachment:
✔️ Boosts confidence working in NHS settings
✔️ Provides reference letters from NHS consultants
✔️ Builds familiarity with documentation, ward culture, and team dynamics
✔️ Can help with interviews, especially if applying in the same trust
Some IMGs even reported getting job offers from the very trust where they did their attachment.
Why Others Are Skipping It
In 2025, a growing number of IMGs are choosing to skip clinical attachments – and here’s why:

🔹 Some NHS trusts accept zero UK clinical experience
🔹 Hospitals are more open to hiring from abroad directly
This doesn’t mean clinical attachments are obsolete. It means they’re no longer a requirement –but rather a strategic choice.
So… Should You Do One?
Ask yourself:
✔️ Do I need NHS references to strengthen my CV?
✔️ Am I applying in competitive regions or specialties?
✔️ Can I afford to spend 4–6 weeks unpaid in the UK?
✔️ Will it help me feel more confident stepping into my first NHS job?
If the answer is yes – it may be worth it.
If not – you can absolutely succeed without one.
Plenty of IMGs are now getting FY2-equivalent jobs, CT1 posts, or even specialty training without ever doing a clinical attachment.
